TAMI HORNER, MD 

Medical Director

Dr. Tami Horner is Board Certified by the American Board of Family Medicine.  She completed her family practice residency training at USF/Morton Plant in Clearwater.  Dr. Horner has undergone specialized training in the treatment of obesity and is a member of the American Society of Bariatric Physicians.  Dr. Horner joined Success by Design as medical director in September 2010 and comes to us with over 8 years experience in medically supervised weight loss.  In addition Dr. Horner has also received fellowship training in  Bio-Identical Hormone Treatment.  She also has received specialized training in aesthetic treatments (Botox/Dysport and Cosmetic Fillers).  Dr. Horner is dedicated to educating her patients on making healthy lifestyle changes including diet, exercise and behavioral changes to reduce risk of cardiovascular disease, diabetes, cancer and other obesity related illnesses.

Sherri ultimately found her passion in disease state management and wellness promotion focused mainly in cardiovascular and diabetes arena.  Her passion for disease prevention was driven by the early death of her father from heart disease and obesity.  Sherri's believes that heart disease and diabetes can be prevented with a healthy lifestyle change. Success by Design was created to help others avoid the complications of obesity. 

Sherri brings over 20 years of nursing, nutrition  and administrative experience. She is passionate about working with patients to ensure success in meeting their personal health and fitness goals.
